[Utr] UPDATE: UTR Project Plan - Common Forms and Binders

reference  - when I sent the whole thing, the email was too big for the list
to accept it - please find attached a .zip file containing all the forms.
 
Your review and comment would be appreciated, preferably by 22Aug09 so I can
re-draft as necessary prior to Investiture.

- Dean Binder divider tabs
 
Forms for Deans - Session Level:
- Dean Session Recap and Expense Reimbursement
- "Dear Instructor" letter (welcome to the session, a few instructions)
- Instructor Envelope Labels
- "Thank You" letters for instructors who offered to teach, but whose
classes were cancelled
 
Forms for Instructors
- UTR Instructor Class Proposal
 
Forms for Instructors - Session Level
- Instructor Evaluation and Expense Reimbursement (printed on yellow paper
so they don't get mixed up with the student evaluations in the instructor's
envelope for each class they teach at a session)
- UTR Instructor Class List (students registered in their classes, used for
signin and pass/fail)
 
Forms for Students - Session Level
- UTR Student Evaluation
- UTR Student Registration (can also be used for Pre-registration)
